{"id":4792,"date":"2024-09-17T15:59:32","date_gmt":"2024-09-17T19:59:32","guid":{"rendered":"https:\/\/moxyinc.ca\/cwoht\/?p=4792"},"modified":"2025-02-03T12:56:37","modified_gmt":"2025-02-03T17:56:37","slug":"what-is-an-ontario-health-team","status":"publish","type":"post","link":"https:\/\/moxyinc.ca\/cwoht\/fr\/what-is-an-ontario-health-team\/","title":{"rendered":"Qu\u2019est-ce qu\u2019une \u00e9quipe Sant\u00e9 Ontario?"},"content":{"rendered":"<p class=\"p1\">Le gouvernement de l\u2019Ontario met en place un syst\u00e8me de soins de sant\u00e9 connect\u00e9 centr\u00e9 sur les patients, les familles et les soignants, appel\u00e9 \u00c9quipes Sant\u00e9 Ontario (\u00c9SO).<\/p>\n<p class=\"p2\">Les ESO organisent et fournissent des services dans les communaut\u00e9s locales. Elles offrent aux personnes des soins mieux connect\u00e9s, int\u00e9gr\u00e9s et plus pratiques.<\/p>\n<p class=\"p2\">L\u2019objectif est de veiller \u00e0 ce que tous les Ontariens puissent b\u00e9n\u00e9ficier de soins mieux coordonn\u00e9s et mieux int\u00e9gr\u00e9s. Au fil du temps, ils travailleront ensemble pour faciliter le passage des patients d\u2019un fournisseur \u00e0 un autre, avec des am\u00e9liorations telles que l\u2019\u00e9tablissement d\u2019un dossier patient unique et d\u2019un plan de soins partag\u00e9s entre les fournisseurs.<\/p>\n<p class=\"p3\">Les OHT sont constitu\u00e9s de fournisseurs et d\u2019organisations, notamment :<\/p>\n<ul class=\"ul1\">\n<li class=\"li4\"><span class=\"s2\">soins primaires<\/span><\/li>\n<li class=\"li4\"><span class=\"s2\">h\u00f4pitaux<\/span><\/li>\n<li class=\"li4\"><span class=\"s2\">soins \u00e0 domicile et en milieu communautaire<\/span><\/li>\n<li class=\"li4\"><span class=\"s2\">services de sant\u00e9 mentale et de lutte contre les d\u00e9pendances<\/span><\/li>\n<li class=\"li4\"><span class=\"s2\">services de soutien communautaire <\/span><\/li>\n<li class=\"li4\"><span class=\"s2\">soins de longue dur\u00e9e<\/span><\/li>\n<\/ul>\n<p class=\"p1\">Les \u00c9SO jouent un r\u00f4le important dans la mobilisation de leurs partenaires pour r\u00e9pondre aux priorit\u00e9s locales, r\u00e9gionales et provinciales. Elles ont d\u00e9montr\u00e9 pour la premi\u00e8re fois leur capacit\u00e9 \u00e0 rassembler des partenaires autour de priorit\u00e9s communes pendant la pand\u00e9mie de COVID-19.<\/p>\n<p class=\"p1\">Ce r\u00f4le demeure important pour faire face \u00e0 de nombreux d\u00e9fis syst\u00e9miques dans la province, notamment la gestion des temps d\u2019attente pour les services et la hausse pr\u00e9vue des maladies respiratoires \u00e0 l\u2019automne.<\/p>\n<p class=\"p2\">Les \u00e9quipes rassemblent des prestataires de soins de sant\u00e9 de tous les secteurs de la sant\u00e9 et des services sociaux pour travailler comme une \u00e9quipe collaborative afin de mieux coordonner les soins et de partager les ressources.<\/p>\n<p class=\"p2\">L&#039;OHT du Centre-Ouest soutient un continuum de soins avec des fournisseurs \u00e0 Brampton, Bramalea, North Etobicoke, Malton et Woodbridge.<\/p>\n<p class=\"p4\"><span class=\"s2\">Apprendre encore plus <a href=\"https:\/\/www.ontario.ca\/page\/ontario-health-teams#section-3\"><span class=\"s3\">ici.<\/span><\/a> <\/span><\/p>\n<p class=\"p5\"><span class=\"s4\"><i>Source: <\/i><a href=\"https:\/\/www.ontariohealth.ca\/system-planning\/ontario-health-teams\"><span class=\"s3\"><i>Sant\u00e9 Ontario<\/i><\/span><\/a><\/span><\/p>","protected":false},"excerpt":{"rendered":"<p>The Ontario government is building a connected health care system centered around patients, families and caregivers, called Ontario Health Teams (OHT).
