This plan will officially launch on April 1st, 2024 and is designed to guide our organization through a transformative journey over the next three years. We are committed to maintaining transparent communication by consistently sharing updates on our progress in achieving the strategic objectives, fostering a culture of accountability, and ensuring our collective success. This dynamic initiative will not only shape the future of our organization but also contribute to the ongoing development and betterment of our community. It reinforces our commitment to current CW OHT priorities while staying flexible to adapt to OHT acceleration, home care modernization, and changing community needs. Aligned with Ontario Health priorities, it emphasizes integrated care, patient navigation, collaborative leadership, and primary care engagement. The plan highlights key initiatives within these areas, offering a focused approach without encompassing all OHT operations.
