They provide people with better connected, integrated and more convenient care.<\/p>\n<p class=\"p2\">The goal is to ensure that everyone in Ontario can benefit from better coordinated, more integrated care. Over time, they will work together to make it easier for patients to move from one provider to another, with improvements such as working towards one patient record and one care plan shared between providers.<\/p>\n<p class=\"p3\">OHTs are made up of providers and organizations including:<\/p>\n<ul class=\"ul1\">\n<li class=\"li4\"><span class=\"s2\">primary care<\/span><\/li>\n<li class=\"li4\"><span class=\"s2\">hospitals<\/span><\/li>\n<li class=\"li4\"><span class=\"s2\">home and community care<\/span><\/li>\n<li class=\"li4\"><span class=\"s2\">mental health and addictions services<\/span><\/li>\n<li class=\"li4\"><span class=\"s2\">community support services <\/span><\/li>\n<li class=\"li4\"><span class=\"s2\">long-term care<\/span><\/li>\n<\/ul>\n<p class=\"p1\">OHTs play an important role in mobilizing their partners to respond to local, regional and provincial priorities. They first demonstrated their ability to bring partners together around shared priorities during the COVID-19 pandemic.<\/p>\n<p class=\"p1\">This role continues to be important to deal with many system challenges across the province.
